An email will be sent confirming receipt of your form and requesting any additional information to facilitate next steps.
You will be required to identify a suitable Specialist to help give you a second opinion.
Once these steps have been completed, you will receive an email with the cost of the second opinion and timeframe.
If you are happy to proceed, payment will be requested before moving forward.
In order to share your medical records with your chosen Specialist, it will be necessary to open a HeartHealth account www.hearthealth.org.uk (Refer to Note 1).
